Please print and fill out the forms. Forms may be faxed, dropped off at
the Parish Office during office hours (Monday - Friday, 8:30 AM to 4:30
PM or Saturday, 9:00 AM to 3:00 PM), faxed to 909.390.9919, or dropped into the Collection
Basket during Mass.
The Blank for
Baptism Register can be turned in at the Baptism Class. For
more information about the Baptism Class and Ceremony please click on
the 'Baptism' button on the top bar.
